We are a Primary Healthcare Organisation who support local GP practices and other health care providers with the Waitemata and Northland areas. One service we would like to promote is DSME. Our DSME team provide free Diabetes Self-Management Education days for people and their whanau with Type 2 Diabetes. They are held at various local community venues. Each day is facilitated by a Nurse Educator and dietary team and includes: catered morning tea, practical workshops, friendly group interaction and support. More details for you from our PDF Flyer: Diabetes Self-Management Education (DSME) helps you better understand diabetes, its causes and how best to maintain/better your health. The day is run in small groups so that you can get the most from it and ask questions, in a safe environment. The day is run by a Registered Nurse Educator and Dietary Team and covers: • Healthy eating: label reading, good and bad fats, food choices • How to stop/control effects of diabetes on the body (eyes, kidney, heart, nerves, feet) • Medications - How do they work? New choices available? • Your blood test results - what do they mean? • Goal setting (action planning/FREE support/where to now?) • How to be the best with your health team You are welcome to bring a family member or friend to support you, kai/refreshments provided.
