Enliven is a service that works alongside you in your home to maximize your independence. We will tailor our services to make sure they are right for you. Once Enliven receives your referral, an Enliven service coordinator will contact you to arrange a visit to your home, hospital or rest home. During the first visit, the service coordinator will: - discuss how you want your support to be organised - help you create a plan that meets your personal goals and service needs. This is so that you: - are supported to achieve your own goals - receive support for personal care and/or household tasks - maintain your independence through a range of activities - stay connected with your community - can be matched with a support worker who is right for you.
